Compound Preperation and Side Effects

 Compound Preparations:

There is neither theoretical nor clinical justification f or the inclusion of other therapeutically active ingredients, such as the B group of vitamins.
Modified Release Capsules and Tablets
These preparations are likely to carry the iron past the first part of the duodenum into an area of the gut where conditions for iron absorption are poor. The low incidence of side-effects may well be because of the small amounts of iron available under these conditions and so the preparations have no therapeutic advantage and should not be used.
Side Effects:
Because iron salts are astringent, gastrointestinal irritation may occur. Nausea and epigastric pain are dose-related but the relationship between dose and altered bowel habit (constipation or diarrhoea) is less clear. Oral iron particularly modified-release preparations may exacerbate diarrhoea in patients with inflammatory bowel disease care is also needed in patients with intestinal stricture and diverticula.
Iron preparations taken orally may have a constipating effect particularly in older patients, occasionally leading to faecal impaction.
